For The Personal Services Of A Disc Jockey

This contract, for the personal services of a disc jockey, made this day ____ /____ /______ between

hereinafter referred to as Employer, and Stan McEvoy and Matt Farr d/b/a Mobile Music Express, hereinafter referred to as Mobile Music.

Mobile Music is employed to entertain at a function termed:

The Employer hereby agrees to retain Mobile Music on the following date:

in the establishment of their choice:

A reserve deposit of $100.00 (One Hundred Dollars) shall be paid by the Employer to Mobile Music at the time of the booking and will be subtracted from the total charge of $_______________________,

payable after Mobile Music has arrived and ready to perform prior to starting the event.

Mobile Music hereby agrees to render such services at the time and place above -- referred to for the amount set forth above. Mobile Music shall enforce disciplinary measures for just cause, and carry out instructions as to selections and manner of performance. The agreement of Mobile Music to perform is subject to proven detention by sickness, accidents, riots, acts of God, strikes or any other legitimate conditions beyond their control. The Employer shall at all times have complete supervision, direction and control over the services of Mobile Music on this engagement and expressly reserves the right to control the manner, means and details of the performance of services of Mobile Music, as well as the ends to be accomplished.

CANCELATION OF THIS CONTRACT SHALL BE MADE NOT LATER THAN THIRTY (30) DAYS PRIOR TO THE ENGAGEMENT. If notice of cancelation is received less than thirty (30) days provided herein, Employer must pay $100.00 (One Hundred Dollars) as liquidated damages for recession of this contract. If Mobile Music cancels for any reason other than those referred to above, Mobile Music must obtain another group which shall provide similar services for Employer under the same terms and conditions as herein above agreed to.
